Mỏi cơ nhai là cảm giác căng tức, mỏi hoặc đau âm ỉ xuất hiện ở các nhóm cơ chịu trách nhiệm cho hoạt động nhai và vận động của hàm. Tình trạng này thường khiến người bệnh cảm thấy hàm nặng, khó chịu, đặc biệt sau khi ăn nhai nhiều, nói chuyện lâu hoặc sau khi ngủ dậy.

Trong y khoa, mỏi cơ nhai được xếp vào nhóm rối loạn cơ nhai (masticatory muscle disorders), thuộc nhóm lớn hơn là rối loạn khớp thái dương hàm (temporomandibular disorders – TMD). Đây là nhóm vấn đề liên quan đến hệ thống gồm cơ nhai, khớp hàm và các cấu trúc xung quanh.

Trên thực tế, các vấn đề xuất phát từ cơ nhai như đau, căng cơ thường gặp hơn so với các tổn thương trực tiếp tại khớp thái dương hàm. Vì vậy, khi nhắc đến rối loạn khớp thái dương hàm, không phải lúc nào nguyên nhân cũng nằm ở khớp, mà nhiều trường hợp lại liên quan chủ yếu đến tình trạng cơ bị quá tải hoặc co căng kéo dài.

Các cơ nhai chính trong khoang miệng

Để hiểu vì sao mỏi cơ nhai có thể gây ra nhiều biểu hiện khác nhau như đau hàm, đau đầu hoặc đau lan vùng tai, trước tiên cần biết đến 4 nhóm cơ chính tham gia vào hoạt động nhai:

  • Cơ cắn (Masseter): Đây là nhóm cơ khỏe và phát triển nhất trong hệ thống cơ nhai, nằm ở hai bên má. Bạn có thể dễ dàng cảm nhận cơ này nổi lên khi cắn chặt răng. Cơ cắn là một trong những nhóm cơ thường bị căng mỏi nhất khi xảy ra tình trạng quá tải cơ nhai.
  • Cơ thái dương (Temporalis): Nằm ở vùng thái dương, kéo dài từ khu vực phía trên tai lên phía bên đầu. Cơ này có vai trò quan trọng trong việc nâng và khép hàm. Do vị trí nằm gần vùng đầu, khi cơ thái dương bị căng mỏi, nhiều người có thể nhầm lẫn với đau đầu thông thường.
  • Cơ chân bướm trong (Medial Pterygoid): Nằm sâu bên trong vùng hàm, có nhiệm vụ hỗ trợ động tác khép hàm và giúp hàm di chuyển sang hai bên khi nhai.
  • Cơ chân bướm ngoài (Lateral Pterygoid): Nằm sâu phía trong hàm, đóng vai trò quan trọng trong việc mở miệng và đưa hàm ra phía trước.

Bốn nhóm cơ này phối hợp liên tục trong các hoạt động hằng ngày như ăn uống, nói chuyện, nuốt nước bọt. Vì phải hoạt động với tần suất rất cao, chúng có thể bị quá tải nếu phải chịu áp lực kéo dài, từ đó dẫn đến tình trạng căng mỏi, đau nhức hoặc co cứng cơ.

Cũng giống như các nhóm cơ khác trong cơ thể, cơ nhai có giới hạn chịu đựng nhất định. Khi phải hoạt động quá nhiều hoặc duy trì trạng thái co cơ trong thời gian dài, các sợi cơ có thể bị căng thẳng và mất khả năng thư giãn hoàn toàn.

Ví dụ điển hình là tình trạng nghiến răng khi ngủ hoặc vô thức siết chặt hàm khi căng thẳng. Những thói quen này khiến cơ nhai phải làm việc liên tục ngay cả khi cơ thể đang nghỉ ngơi, lâu dần gây ra cảm giác mỏi, nặng hàm hoặc đau âm ỉ.

Điều trị u men răng
